Download Forms
- To download:
- Click on the button(s) below and save the two (2) forms to your desktop.
- Complete the forms in Microsoft Word.
- Submit them to the registrar by uploading in "Submit Forms" below.
- To upload:
- Click "Choose File"
- Select the file from your desktop and click "Open"
- The title of the doc file will appear next to the "Choose File" button below
- After uploading both forms and typing optional "comments," click "Submit"
- "Thank you. Your information has been submitted." will appear after a successful submission.
