- To DOWNLOAD:
- Click "Download file" and save the form(s) below to your desktop.
- Complete the forms in Microsoft Word.
- Submit them to the counselor by uploading in "Submit Scholarship Applications" below.
- To UPLOAD (submit):
- Click "Choose File" under "Upload Scholarship Application" below
- Select the file from your desktop and click "Open"
- The title of the doc file will appear next to the "Choose File" button below
- After uploading form and typing optional "comments," click "Submit"
- "Thank you. Your information has been submitted." will appear after a successful submission.
- You must submit each application separately if applying for more than one scholarship.
- To do so, refresh the page in your web browser.
